By submitting this form I understand I may get support in returning with my pet but I am still the person responsible for my animal. I guarantee that I will collect my animal within 2 hours of their arrival in Yellowknife. Animals not picked up will go to a boarding facility and will result in boarding fees that I will be required to pay.

I hereby waive, release, and discharge Veterinarians Without Borders, NWT SPCA, the airline, the City of Yellowknife and the Government of the Northwest Territories, and their agents, officers, employees, or volunteers from any and all liabilities that result in injury to, or the loss of my dog(s), or any other property of mine which arises in any way out of the transportation

I understand that dogs can sometimes be injured or become ill during transportation. I accept that Veterinarians Without Borders, NWT SPCA, the airline, the City of Yellowknife and Government of the Northwest Territories are not in any way responsible for any injury, stress or illness my dog(s) may suffer while being transported. I will not hold any party liable, financially or legally, for any physical, mental, or health issues that arise during or because of my dog’s transportation. This includes, but is not limited to: dog bites, minor cuts and scratches, bone breakage, tissue damage, joint problems, stress, or diarrhea.
